PACE Physical Therapist

Essential Responsibilities:

  • Provide Physical Therapy treatment and interventions to PACE participants across the continuum, including in the home and/or community setting.
  • Conduct comprehensive assessments of participants to determine their functional abilities and needs.
  • Actively participate in the interdisciplinary team by offering clinical expertise in team planning, problem-solving, and processes.
  • Develop and implement personalized care plans aimed at improving or maintaining participants’ independence and quality of life.
  • Guide participants in therapeutic, creative, and self-care activities to enhance their strength, mobility, and independence.
  • Review participants’ progress and adapt treatment plans as needed.
  • Provide training, guidance, and supervision to Physical Therapy Assistants (PTA) or Personal Care Assistants, and participant/family members in carrying out the care plan.
  • Collaborate closely with other healthcare professionals, including physicians, nurse practitioners, physician associates, nurses, physical therapists, social workers, and other members of the interdisciplinary care team, to provide integrated care.
  • Meet all administrative requirements, including attendance at team and staff meetings, and recording and tracking PT rehabilitation equipment.
  • Visit participants in their homes or care settings as needed.
  • Serve on various internal and external committees as required or designated.
  • Provide all services in a manner consistent with State and Federal regulatory requirements.
  • Participate in team meetings, caregiver groups, and special events as needed.
  • Visit participants in their homes or care settings.
  • Document in the electronic health record accurately, professionally, and in a timely manner.
  • Foster a safe and supportive environment conducive to the professional development of healthcare professionals.

Supporting Responsibilities:

  • Meet department and company performance and attendance expectations.
  • Follow privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.
  • Perform other duties as assigned.

SUCCESS PROFILE

Work Experience: A minimum of 3 years of experience in various healthcare settings is required, preferably in care management, geriatrics, chronic disease management, home health, or long-term care. Additionally, 1 year of experience working with frail, older adults is required.

Education, Certificates, Licenses: A Bachelor’s degree from an accredited physical therapy program and PT licensure is required. CPR certification (BLS or ACLS) is required. A valid driver's license is required.

Knowledge: Employee(s) must have the ability to work effectively with individuals who have diverse health issues, concerns, and personalities. They should be proficient in accurately documenting case notes, assessments, contacts, and writing letters that may become part of legal records. Excellent organizational skills and experience with various computer software applications, including Word, Excel, PowerPoint, and Outlook, are essential. The capability to work independently with minimal supervision is required, along with exceptional communication skills, both verbal and written. Strong active listening skills are also necessary.

Each member of the PACE organization's staff that has direct contact with participants must meet the following conditions:

  • Be legally authorized (for example, currently licensed, registered or certified if applicable) to practice in the State in which he or she performs the function or action.
  • Only act within the scope of his or her authority to practice.
  • Have 1 year of experience working with a frail or elderly population must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
  • Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.
  • Be medically cleared for communicable diseases before engaging in direct participant contact.
