PACE Social Work Supervisor
Essential Responsibilities:
- Scale, grow, and lead a team of social work professionals as the PACE organization progresses
- Actively participate and collaborate as a member of the Interdisciplinary Team, including care planning.
- Provide leadership to PACE social workers to ensure high-quality care. Serve as a mentor to enhance the knowledge and skills of the social work team and act as a clinical supervisor to MSWs.
- Conduct orientation and training for new hires. Provide ongoing supervision, training, coaching, evaluation, and leadership to assigned team members, including annual reviews, promotions, and terminations.
- Ensure all services are provided in a manner consistent with State and Federal PACE regulatory requirements and that the team understands these requirements.
- Incorporate knowledge of comorbidity and established standards of practice associated with medical and behavioral health diagnoses.
- Accurately and completely document in the electronic health record in a timely manner.
- Serve as a resource to assist participants and their families in accessing services, facilities, and community resources for their healthcare needs.
- Establish and maintain relationships with community services, and internal and external referral sources.
- Provide on-call coverage for direct patient care with other staff as needed.
- Identify and designate ongoing training needs for social work staff and develop/implement appropriate educational offerings.
- Ensure exceptional customer service to participants of the PACE program.
- Report on and collaborate to resolve participant grievances.
- Work collaboratively with other PACE leaders to maintain and develop departmental communication materials, training/educational materials, and policies and procedures.
- Actively participate in various strategic and internal committees to disseminate information within the organization.
- Actively participate in efforts to support compliance and regulatory requirements across lines of business.
- Coordinate business activities by maintaining collaborative partnerships with key departments.
- Monitor, evaluate, and act on team assignments relating to volume, timelines, accuracy, customer service, and other quality and performance measures as appropriate.
- Foster a safe and supportive environment conducive to the professional development of healthcare professionals.
Supporting Responsibilities:
- Meet department and company performance and attendance expectations.
- Follow privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.
- Perform other duties as assigned.
SUCCESS PROFILE
Work Experience: A minimum of 5 years of social work experience, including at least 1 year of leadership experience and 1 year of experience working with a frail or elderly population, is required. Experience in PACE or other Long Term Services and Supports settings is preferred.
Education, Certificates, Licenses: A Master’s degree in Social Work (MSW) is required. A Licensed Clinical Social Worker (LCSW) in the State of Oregon is preferred. Valid CPR certification required. A valid Oregon driver’s license is required. Relevant behavioral health care experience is preferred.
Knowledge: Employee(s) must have knowledge of behavioral health State Medicaid and Medicare regulations. Strong analytical and problem-solving skills are essential. Proficiency with computer information systems and software is required, along with basic statistics and research methods.
